Financial Aid Forms

Most financial aid forms can be submitted electronically. Follow the steps below to set up your StudentForms account.  

1. Go to https://roosevelt.studentforms.com/ and login using your Roosevelt NetID and password
2. Click ‘Create Account’ link
3. Enter/confirm your student information; make sure this matches what you provided on the FAFSA.
4. Click ‘Create Account’ button
5. Be sure to enable both text message and email notifications

Once your account is created, any required tasks you need to complete will become available.
