Creating a Cohort:  New Cohort Approval Form

 

Please click here to request a New Cohort Approval Form to be emailed to you.

 

This form must be filled out by the Program Director or Department Chair, and submitted via email as an attachment for approvals. 

 

Approvals Required:

  • The Dean's - to be obtained by the Program Director/Department Chair
  • Provost's - to be obtained by the Dean
  • VP for Finance - to be obtained by the Provost

After all required approvals are completed for the new cohort, the Provost Office will notify (via email) the offices listed below:
