Equipment Request Form (Non-classroom Event)
Multimedia Services | ITS

Use this form to request equipment for non-classroom events (lectures, concerts, meetings, conferences, etc.). All such events require MMS staff to operate equipment. Most non-classroom events are not included in the MMS budget and must be paid for separately through an IRT from your department's budget. To submit requests for classes, use the classroom equipment request form.

Important note: many Roosevelt classrooms now contain built-in display and projection equipment. For information on using computers in these spaces see the classroom computer policy.

Equipment requests for special events must be made at least two weeks in advance (even more lead time is greatly appreciated).  Also, please send MMS a copy of your room layout.
