General Compliance Statement

This section of Insurance Plans contains fair and complete summaries of the material terms of each of the benefit plans available to you as an employee of Roosevelt University. The summaries include general descriptions of eligibility and coverage offered, methods of election to participate and employee costs. Detailed copies of each plan can be obtained from the Department of Human Resources at 312-341-4331.

The benefit plans in this section are administered under legally enforceable conditions; that is, the plans do not permit, grant, or deny participation coverage, or benefit rights through exercise, directly or indirectly, of discretion on the part of Roosevelt University, the plan administrators, fiduciaries, trustees, or other persons. Once the conditions for an employee to participate in a plan have been met, that employee may participate in that plan.

The plans in this section are maintained for the exclusive benefit of eligible Roosevelt University employees and retirees, their spouses and dependents, and to COBRA qualified beneficiaries.

Roosevelt University intends to maintain each of the plans in this section for an indefinite period of time. However, the University reserves the right to modify or terminate a plan, or choose not to renew a plan pursuant to a specific plan provision. A change in carrier or health care provider will not be considered a modification or termination as long as the coverage and benefits have not substantially changed.

ERISA Rights

As a participant in the Roosevelt University benefit plans, you are entitled to certain rights and protections under the Employee Retirement Income Security Act of 1974 (ERISA).

You must apply for all insurance benefits

Coverage is NOT automatic; you must complete an application for each insurance or retirement benefit at the time you become eligible. The University will apply on a timely basis for any benefit available to you. The University reserves the right to change, modify, or discontinue any of its fringe benefit programs at any time.
