Long-Term Disability

Faculty and administrative employees are eligible for this fully-funded insurance plan, as of the first day of October following one (1) year of continuous, full time service at Roosevelt University *. New employees who had comparable coverage with their previous employer for at least one year may have this eligibility period waived with proof of previous coverage.

After 180 days of qualified, continuous absence under the University’s short term disability benefit plan (also known as the “elimination period”), you may apply for long term disability. Intermittent absence due to illness or injury may not be eligible for paid leave under this policy. The benefit for qualified, long term disability is equal to 60% of your monthly salary as of the date of disability began, with a limit of $6,000 per month. Your payment may be reduced by deductible sources of income and disability earnings. Some disability may not be covered or may have limited coverage under this plan. The minimum monthly payment is the greater of $100 or 10% of your gross disability payment.

If you are disabled due to a sickness or injury and you lose the ability to safely and completely perform 2 or more “Activities of Daily Living” without another person’s assistance or verbal cueing then the insurance carrier will pay you 20% of monthly earnings to a maximum monthly benefit of the lesser of the LTD plan maximum monthly benefit or $5,000. The “Activities of Daily Living” include bathing, continence, dressing, eating, toileting and transferring.

The maximum duration of benefit payments under this plan is dependent on your age and the type of disability.

The University’s long term disability insurance carrier determines your medical eligibility for benefits based on required information provided by you and your attending physician(s). This insurance carrier, not Roosevelt University, reviews all disability information and then informs the University if you are eligible for qualified long term disability payments. If you fail to provide the required information for eligibility determination in a timely manner, related absence will be considered unexcused and may result in separation of employment.

If you are qualified for a long term disability leave, you are responsible for paying the total premium for all fringe benefits in which you are enrolled. University contributions to your retirement plan will discontinue while on qualified long term disability. You may not be eligible for tuition remission while on long term disability leave.

If you must go out again due to the same serious health condition, you may re-apply for long term disability benefits immediately if your recurrent disability occurs within six (6) months of the end of your prior claim.

The maximum period that you may remain a Roosevelt University employee while on a qualified, long term disability leave is 540 days (18 months). If separation of employment occurs, you may be eligible to continue to receive long term disability benefits as defined under the insurance policy. You may continue your health and dental coverage as allowed under COBRA regulations and other insurance plans may be converted to individual policies if available.
