Application for Field Experience

The application form on the linked page (see link at bottom of this page) is sent to various schools as the Office of Student Services seeks placements for you, so it is important that it is accurate and clear. Please submit one copy of this form for each field experience placement. If you have more than one placement in a term, you will fill out multiple forms. To do that easily, fill out the form once, save it to a disk, flash drive, or hard drive with the name of the first course (see directions below). For a second course, click File, Save As and use a different title (again, see directions). You can even save the completed form and use it in a subsequent semester simply by updating the information

Instructions: The form on the following page has shaded fields which you will fill in. Using the form is relatively simple:

To access the form, click on the link at the bottom of the Instructions. You may want to print this page so that you have a copy once the Application opens.
Tab between fields in the application and either choose from the drop-down list, check a box, or enter text.
Note that some of the text fields limit the amount of text you can enter; plan ahead and/or edit carefully so that you can include all that you want. When you finish, save the application with the filename LastnameFirst-Course (e.g., AndersonScott-ELED421). Attach the file to an e-mail message with the words, Field Experience, in the subject line and send to placements@roosevelt.edu
You will be able to use this application for other semesters simply by updating the information.
When your application is received, you will receive an automatic reply.
After that automatic reply, all e-mail from the College’s Office of Student Resources will be sent to your RU e-mail address. Please activate the address if you haven’t done so, and check it regularly.
