Software Installation Request Form

Instructors: Use this form to request the installation of special software not listed on the standard software page. The following policies and procedures apply:

• Requests for fall semester 2008 must be submitted by May 9, 2008 at the latest to ensure availability by the start of fall 2008 classes. Late requests submitted before the start of the semester will be addressed beginning the third week of the semester, and requests submitted after the start of the semester from 3 to 5 weeks after the date of the request. The actual time required will depend on the complexity and scope of the request.

• All software CDs/DVDs should be delivered to Matt Anderson in Auditorium 264, along with installation documentation and licenses and/or copyright permission from the software vendor/publisher to cover the number of machines to be installed. Software delivered without proper documentation cannot be installed.

• Purchase of special-purpose software will be charged to the budget of the requesting department. DoIT cannot purchase or license software without approval from the appropriate budget manager.

• MS Windows software must be Vista compatible.

• Faculty requesting the installation of applications not purchased by DoIT must be able to provide proof of license.

• Trial or evaluation copies of software cannot be installed for student use.
